چکیده

Purpose: To assess the effects of micronized (MIC) and nanoparticulate (NANO) transdermal hormone therapy (THT) on blood pressure, fasting insulin and ultra-sensitive C-reactive protein (CRP) in postmenopausal women. Methods: In this open label study, 27 postmenopausal women, with no clinical evidence of cardiovascular disease, were randomly divided in two groups. During 12 weeks, fifteen patients received on the left forearm micronized (MIC) THT (micronized 17β-estradiol 2.5 mg/ day + progesterone 100 mg/day). Fourteen patients received a nanoparticulate (NANO) THT (nanoparticulate 17β-estradiol 2.5 mg/ day + progesterone 100mg/day). After treatment patients were evaluated. Baseline and Post-THT measures were determined: Insulin, body mass index, waist circumference, blood pressure, CRP-stratified levels, total progesterone, TSH and FSH levels. The study was registered at clinical trials.gov (NCT02467673). Results: The mean age was 61.4 years. CRP-stratified high levels decreased in a higher number on NANO THT patients, who moved from high to intermediate and low levels (P<0.05). No effect was observed on TSH and FSH serum levels in both groups. Insulin levels decreased significantly on NANO group (P<0.05) patients and did not change on MIC group. Conclusions: Micronized and Nanoparticulate THT for 12 weeks had beneficial effects in postmenopausal women with no clinical evidence of cardiovascular disease.

Postmenopausal hormone replacement therapy and cardiovascular disease: the value of transdermal estradiol and micronized progesterone.

Most available postmenopausal hormone replacement therapies (HRT) offer similar efficacy, but differ with respect to the cardiovascular risks associated with their use. There is a wealth of evidence to suggest that, unlike oral estrogens, transdermal estradiol does not increase the risk of venous thromboembolism, probably due to its lack of effect on the coagulation cascade, including thrombin ...

Activated protein C resistance among postmenopausal women using transdermal estrogens: importance of progestogen.

OBJECTIVE Although the route of estrogen administration is known to be an important determinant of the thrombotic risk among postmenopausal women using hormone therapy, recent data have shown that norpregnane derivatives but not micronized progesterone increase venous thromboembolism risk among transdermal estrogens users. However, the differential effects of progesterone and norpregnanes on he...

Aortic hemodynamics in postmenopausal women following cessation of hormone therapy

Central (aortic) blood pressure and aortic pulse wave characteristics are measures of cardiovascular health, predictive of cardiovascular mortality. Previous studies have compared aortic hemodynamics in women who do and do not take menopausal hormone therapy, but characteristics of these parameters following cessation of treatment have not been defined. Therefore, the purpose of this study was ...

Nanostructured transdermal hormone replacement therapy for relieving menopausal symptoms: a confocal Raman spectroscopy study

OBJECTIVE To determine the safety and efficacy of a transdermal nanostructured formulation of progesterone (10%) combined with estriol (0.1%) + estradiol (0.25%) for relieving postmenopausal symptoms. METHODS A total of 66 postmenopausal Brazilian women with climacteric symptoms of natural menopause received transdermal nanostructured formulations of progesterone and estrogens in the forearm ...

Estradiol therapy combined with progesterone and endothelium-dependent vasodilation in postmenopausal women.

BACKGROUND Epidemiological studies indicate that estrogen replacement therapy decreases the risk of cardiovascular events in postmenopausal women. Estrogen may confer cardiovascular protection by improving endothelial function because it increases endothelium-dependent vasodilation. It is not known whether progesterone attenuates the beneficial effects of estrogen on endothelial function. MET...
